We are seeking a Physical Therapist to join our Lynchburg – Lovingston team. We offer diverse specialized services, including Back Pain, Concussion, Vestibular/Vertigo, Dry Needling, Foot and Ankle, Geriatrics, Head, Neck, and Facial Pain, Health and fitness, Industrial Rehabilitation, Local School Sports, and Manual Therapy. Responsibilities
- Assesses patients and develops and initiates treatment plans based on the evaluation results.
- Review the Physician’s referral (Prescription) and the patient’s condition and medical records to determine the physical therapy treatment required.
- Provides direct patient care by established protocols and patient care guidelines.
- Documents findings, progress, and instructions to patients and caregivers.
- Responsible for patient care performed by assistants and rehabilitation technicians.
- Completes a written plan of care, develops goals based on evaluation findings, and develops a comprehensive program to attain goals.
- Implements physical therapy treatment program and provides essential physical therapy treatments.
- Communicates effectively with patients/caregivers and professional colleagues, including physicians and insurance companies.
- Ensure timely documentation, including daily notes, weekly workers’ compensation reports, progress reports, initial evaluations, discharge summaries, and incident reports.
Requirements
- Graduate from CAPTE-accredited Physical Therapy program.
- Current state of Virginia license or ability to obtain Virginia license and CPR certification.
- Able to create and implement outpatient balance and vestibular rehabilitation-setting treatment plans.
- Ability to work efficiently/effectively in an autonomous environment.
- Possesses basic PC skills and knowledge, including but not limited to Electronic Medical Record (EMR) systems, Database software, and MS Office.

Provide direct nursing care to patients undergoing diagnostic, therapeutic and interventional procedures in the Cardiac Catheterization Lab.
- Assess, prepare and position patients for procedures. Perform conscious sedation and hemodynamic monitoring as necessary.
- Prepare, implement and document plan of care. Communicate patient’s condition to physician before, during and after procedures.
- Perform and/or facilitate diagnostic, therapeutic and interventional procedures.

Responsibilities
Provide direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.
- Implement and monitor patient care plans.
- Monitor, record and communicate patient condition as appropriate.
- Serve as a primary coordinator of all disciplines for well-coordinated patient care.
- Note and carry out physician and nursing orders.
- Assess and coordinate patient’s discharge planning needs with members of the healthcare team.
Requirements
Education:
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
- Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
Education:
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.

Avera is currently seeking Physical Therapist (PT) looking to deliver the best possible care in Avera’s patient-focused settings.
Job Summary
Administers physical therapy treatments on in-patients, outpatients, swing bed patients and nursing home residents and pediatrics population in accordance with facility policies and accepted standards of practice for physical therapy to assist in the rehabilitation and restoration of body functions after illness and/or injury.
Education and/or Experience
- Graduate from an accredited physical therapy program of the American Physical Therapy Association preferred
Licensure/Certification/Registration
- Licensed by the State of Iowa as a Physical Therapist required.
- Completion of the Child Abuse Dependent Adult Abuse courses for Iowa Mandatory Reporters within 90 days of employment and every 3 years thereafter.
Work Schedule
- Monday-Friday, 8:00am-4:30pm (negotiable); 80 hours per 2 weeks
Pay
- $35.00 – $53.00 per hour
